THERAPY AS AN INVESTMENT
While outpatient counseling is often covered entirely by health insurance, many policies do require you to shoulder some of the costs. Know that it is a bargain. Outpatient counseling is a great tool for achieving your family, work and educational goals.
Unresolved personal issues jeopardize careers, causes failure in the classroom and creates unhappy homes. Many issues are easily resolved in just 6 – 8 sessions, making outpatient therapy one of the best investments you can make for you and your family.

We've provided our information forms online so you can fill them out before you arrive at our office. You can download them here to print and complete. There are five forms below for you to download and complete.
If you don't want to fill out the paperwork ahead of time, plan to arrive at our office at least 15 minutes before the appointment to complete the forms in person.

Contact information for any other healthcare providers you want us to coordinate your care with.
You will only need this information for the first appointment, unless the information changes.
If you have insurance, we will bill the insurance provider, if we are able to verify coverage at the time of your first visit. If not, we will expect full payment from you. You are responsible for any portion not covered by your insurance. However, if your coverage is a contracted HMO or PPO, you are only responsible for the deductible and/or co-payment. Other contracted services will be billed according to the contract provisions.
Therapists at Therapy Resource Associates are on numerous preferred provider, EAP, and HMO panels.
Therapy is a commitment of your time, energy, and money. We request you make an appointment in advance and notify us 24 hours in advance if you need to cancel or reschedule. Emergencies do arise, but we reserve the right to charge you for failure to cancel within 24 hours of the appointment and/or failure to arrive for the scheduled appointment.
OTHER SOURCES OF PAYMENT
Other Insurance Policies
If you have insurance other than the above, you can check if you have "out-of-network" coverage. This is typically provided with PPO plans. Additionally, some clients find it helpful to pay for therapy using a Health Savings Account (HSA).
Workers compensation policies
If you were injured at work, you may be eligible for compensation from your employer’s “work comp” policy.
Victim's compensation
If you or your dependent has been a victim of a crime and a police report has been filed, you may be eligible for Victim’s Compensation funds.
FILING CLAIMS
We will file your insurance claims for you. Benefits from your policies will be paid directly to Therapy Resource Associates, and we will apply those payments to your account.
If you feel that a claim is processed incorrectly, you will need to contact your insurance company for a resolution of the problem.
